Requests may be submitted in person, by mail, by fax, or by E-mail. You may request your records in person, Monday through Friday from 9:00 am to 12:00 pm and 1:00 pm to 4:00 pm.
If you need to communicate by mail or fax, please download and complete the Authorization for Release of Health Information Form and fax to (858) 534-7545.

Medical records sent to SHS from an outside provider or facility should be mailed to the above address.
We only maintain records of immunizations that you acquire at SHS or records you personally send to us to file in your medical chart.
Lab results are available for pick up in the Medical Records Department only after review and approval by one of our providers. There is no charge for this service, however, a valid picture ID must be presented at time of request.If you need an explanation of the lab results, you should contact the provider who ordered the tests.
